MCQ 10 (Medicine/ bee stings

Patient with bee sting. The family is concerned that the stinger may still be in the patient’s skin. The proper response is to:
A) Remove the stinger with forceps.
B) Remove the stinger using a double-edged razor or credit card.
C) Leave the stinger embedded in the skin.
D) Not worry about looking for a stinger in this patient.

Discussion
The correct answer is D. The one Hymenoptera (bees, wasps, etc) that leaves the stinger in the skin is the honeybee. Because this patient was stung by a yellow jacket, there will be no stinger to find in this child. As for the other answers, either A or B would be appropriate treatment if a stinger was present. The only thing that seems to make a difference in the amount of venom injected is the length of time the stinger is in the skin and not the mechanism by which is removed. So, remove the stinger as quickly as possible by whatever means available.
